Your First Visit
Paper Work
Your first appointment will probably last no more than an hour. When visiting Westside Family Health Center for the first time, we ask that you arrive about 15 minutes before your appointment to fill out paperwork. Please be prepared to answer some brief health history questions.
You may also download some of the forms, print and complete them ahead of time. Bring them the day of your appointment. Do not sign and date these forms until you speak with one of our medical staff on the day of your visit.

Parking

Landmarks
Westside Family Health Center is located on the north side of Ocean Park Boulevard between 17th and 18th streets, across the street from John Adams Middle School and Bob’s Market.
What to Bring
- If you have Insurance and/or Medi-Cal, please bring your insurance card with you along with a picture ID.
- Bottles of any medication you are currently taking.
- If it is a pediatrics visit please bring your child’s immunization record to every visit.

Call (310) 450-2191. Our trained Call Center team will assist you with any questions and schedule an appointment for you. Call Center hours: Monday-Friday, 8:30 a.m. to 5 p.m. and Saturdays, 9 a.m. to noon.

Walk-ins are welcome
Our address is 1711 Ocean Park Boulevard, Santa Monica, CA 90405. We are located on the north side of Ocean Park Boulevard between 17th and 18th streets.
We are open 55 hours each week for your convenience:
- Monday and Thursday, 8:30 a.m. to 6 p.m.
- Tuesday and Wednesday, 8:30 a.m. to 8 p.m.
- Friday, 8:30 a.m. to 4 p.m.
- Saturday, 8:30 a.m. to 2 p.m.
- Sunday, closed
And, we are open on certain holidays including Martin Luther King Day (January), President’s Day (February), Columbus Day (October) and Veterans Day (November) for all services. Please refer to our Calendar or call (310) 450-2191.
