Westside Family Health Center (WFHC) accepts all patients regardless of their insurance or financial status.

Our federally funded healthcare program allows WFHC to offer a wide range of services to patients through a sliding-fee scale. We also take Medi-Cal and various forms of insurance. What to Bring if You’re Applying for Health Coverage

For all programs, you will need:

  • Proof of income (pay stub, unemployment insurance stub, income tax return, Social Security award letter, a letter from your employer if you are paid in cash*)
  • Government-issued Photo ID (State ID, Driver License, Passport, Military ID card, etc.)
  • Proof of address (such as a utility bill with your name on it, or a driver license*)


If you have them, you should also bring:

  • Birth certificate
  • US residency card
  • Automobile registration
  • Social Security card
  • Proof of pregnancy

* For My Health LA only, you can also issue an affidavit if you have no other proof of income or address.

If you are not a U.S. citizen, you are still encouraged to bring in proof of income and address verification as you may be eligible for a variety of no-cost or low-cost health care programs.

These documents are used solely for determining your eligibility for health care programs and will NOT be submitted to the US Citizenship and Immigration Service (USCIS) or Internal Revenue Service (IRS).

Is My Health LA Right for you?

  • You can keep WFHC as your medical home
  • We can help with enrollment and re-enrollment


You may qualify for My Health LA if all of the following apply:

  • You live in Los Angeles County.
  • 26-29 years of age.
  • Monthly income no more than $1,437 for a family of one (if your family is larger, the threshold is higher).
  • Not pregnant or eligible for Medi-Cal.

Do You Qualify for Medi-Cal?

Even if you’ve been turned down in the past, the Affordable Care Act has made more people eligible for Medi-Cal.

You may be eligible if:

  • You are a legal resident of California (or have refugee status).
  • Your income is less than $1,437 for a family of one (check here for all family sizes).
  • You are under 25 or 50 and over.
  • You are pregnant.
  • You are blind or disabled.
  • You are already enrolled in Cal-Fresh, Cal-WORKs, SSI/SSP, Refugee Assistance, or Foster Care/Adoption Assistance Program.
