Primary Responsibilities

The In-Reach Patient Care Coordinator will provide in-reach and case management services to patients who are enrolled in Medi-Cal and Medicare programs helping them navigate through health care services and systems. Promoting the importance of wellness. The In-Reach Patient Care Coordinator will work closely with the patient care teams to support positive health outcomes. The In-Reach Patient Care Coordinator is the main point of contact for patients and removing the patient’s barriers to care by assisting them to make appointments for wellness visits.

Qualifications
  • Two or more years of experience in customer service or one or more years of experience in a healthcare setting and prior experience in a health plan, medical group or IPA setting working with physician networks
  • Ability to convey a positive and professional image while maintaining contact with focus with audience.
  • Self-motivated, flexible and a team player, must have the ability to work independently, yet know when to ask questions.
  • Must be well organized, have excellent phone/people skills, and be able to handle multiple tasks.
  • Must have strong computer skills and effective communication skills a plus, especially in Microsoft Word, Outlook and Excel.
  • Effective verbal and written communication skills; strong computer skills desirable.
  • Available to work evening and Saturday.
  • Bilingual English/Spanish is preferred.
Education College or other post-secondary education preferred or work equivalent experience.
Salary and Benefits Salary is negotiable and commensurate with education and experience. WFHC offers a full range of benefits including a 403(b) plan.
