Your First Visit

Your first appointment will probably last no more than an hour. When visiting Westside Family Health Center for the first time, we ask that you arrive about 15 minutes before your appointment to fill out paperwork.

For your safety, masks are required for all visitors and patients.

Helpful things to know

Please be prepared to answer some brief health history questions.

You can download several of the forms, and print and complete them ahead of time. Bring them on the day of your appointment. 

Do not sign and date these forms until you speak with one of our medical staff on the day of your visit.

What should you bring to the center?

If applicable, bring your health insurance and/or Medi-Cal card along with a picture ID, and bottles of any medication you are currently taking. If this is a pediatrics visit, bring your child’s immunization record to every visit.

Westside Family Health Center has a Patient Portal that allows you to track and schedule appointments, receive appointment reminders, see lab results and more. If you don’t already have a patient portal account with WFHC you can create one here.
